Hardly inspires confidence but hardly a huge surprise either. The NHS like everything else is busted.

I'm afraid that no amount of money will change human nature. When a lone patient fails to answer their name after a long wait to get some treatment, the staff will assume they have gone home. 

If this had been a railway accident instead of an NHS incident, and enquiry would be held and recommendations made with a view to ensuring that no similar incident would happen again, so maybe we need an independent NHS Incident Investigations Branch.

Perhaps if they got the cleaners to poke anybody laying on the floor to make sure that they haven't died waiting?

Perhaps if they had chipped sensors in wrist bands issued to patients as they arrive which could locate  the whereabouts of anyone who is still on site.  Heaven forbid that they organise pagers or pager apps so mobile patients can go walkabouts (eg to the cafe) and receive instructions to go to a certain place at a certain time when a slot/bed becomes available.
