Not having seen his file it's hard for me to offer an actual explanation.

Usually in these cases there has been collusion by corrupt DWP staff, also usually having non-British ethnicity.

The corrupt DWP officer will tick checks as being done when in fact they haven't.

If the fraud doesn't involve DWP staff collusion, the only other method would be for the fraudster to obtain 5 sets of convincing identity documents & addresses which would be quite some task.

Also he'd need adresses that would cover for him when a DWP Compliance Officer visited & he wasn't there.

Can't imagine there are many cases of such complexity.  The bit that really pees me off is that he'll never be made to pay it back!
